Submit a request on the LA Homeless Outreach Portal (lahsa.org/portal/apps/la-hop/request), choosing 'self-referral' if it is for you. Or call 211, or email pathlosangeles@epath.org.

PATH runs outreach, interim housing and permanent housing for people experiencing homelessness in LA County. To reach PATH, request outreach on LA-HOP or call 211, which routes you to PATH if you live in its service area.
